Alert not created in SAP CRM Webui

Hello Gurus,

I assigned the action profile to my lead transaction, however, when I save a lead, an alert is not generated.  I believe all the following steps were completed correctly:

1. Create alert category

2. Assign authorization

3. Copied BADI Trigger_Alert and changed the the associated code

4. Create new action profile and new action definition

5. Subscribed to the alert in my worklist

6. Assigned action profile to transaction YLD - Lead

I've also tested using the report RSALERTTEST and the alert is sent however when creating a lead transaction, nothing happens.  What am I missing?
