Empty Sreen appears "select Organization Attributes -- Web Dialog" , but "Distribution channels" can not be selected, because the screen is empty.

When I am creating Opportunity i am getting a POPUP window  Select Organization Attributes  but in that window empty Scrren is coming to Select Distribution Channels .Please suggest me how to achieve this requirements.
